Assessment of Your Lockout Tagout Current State

This tool is the starting point for all LOTO improvement to come

The scale is numbered, and color coded from 0 to 5. A score of 0 (or red) represent a weak or critically low LOTO maturity level. In contrast, a score of 5 (or green) represents a perfect score and a hight LOTO maturity level.

#1 Is your lockout tagout program written accurately including all mandatory requirement? (A score of "5" requires all specific hazardous situations are covered and managed).

#2 Number of incidents and/or accidents in the past 2 years? (5 incidents and more is scored as a 0. No incident or accident is scored a 5)

#3 Lockout tagout related incidents or accidents have been analyzed for root causes and appropriate corrective action identified.

#4 Corrective actions are completed within and established timeline.

#5 A zero tolerance policy is documented and applied with rigor within the organization.

#6 All equipment and isolation points are registred, tagged, and standardized across all departments and facilities.

#7 All equipment and isolation points are physically identifieds and standardized to eliminate mistakes and confusion.

#8 Specific lockout tagout documented procedures are available for each machine and/or hazardous situation.

#9 Lockout tagout procedures reflect all types and hazardous energy sources (mechanical/electrical/chemical/pressure, etc.).

#10 Lockout tagout procedures reviews are planned and conducted annually to ensure accuracy.

#11 Approved high voltage work and lockout tagout procedures are present and utilized.

#12 Adequate lockout tagout equipment (boxes, padlocks, and accessories) are readily available effective and in sufficient quantity for workers.

#13 Padlocks used by each worker for lockout tagout are single key models.

#14 Contractors are fully trained and informed on lockout tagout policies and procedures.

#15 Lockout tagout training is provided to all employees combined with a comprehension evaluation.

#16 Lockout tagout refreshner training is conducted on a regular basis and included a comprehension evaluation.

#17 Workers are informed and trained on modifications and changes towards lockout tagout practices.

#18 Internal LOTO audits are conducted according to established timelines and objectives.

#19 External LOTO audits have been conducted over the past three years.

#20 Audit non compliances are tracked and corrected within defined timelines.
